How To Apply
Requirements
Applicants must be at least 18 years of age by the end of the calendar year in which they apply. All applications must be made on official forms provided by the program and must include:
1. A completed application form. (pdf document- requires Adobe Reader)
2. OFFICIAL High School Transcript or GED. No copies!
3. ALL College / Post - Secondary School Transcripts documenting all the required five prerequisites.
4. $25.00 non-refundable application fee.
5. SAT Scores (if available)
6. Evidence of a minimum of 8 hours of self-scheduled job shadowing in a medical imaging department or medical imaging clinic documented on facility letterhead.

Deadline for application is Jan 31 of each year for the session beginning in September. Applications received after the Jan 31 deadline will be returned. No exceptions will be made.
All applications will be reviewed by the Admissions Committee. An entrance examination will be administered in February to all applicants who meet the requirements. Those passing the exam and meeting additional criteria will be offered a personal interview. Members of the Admissions Committee are the Program Director, Clinical Instructor, Supervisor of the Department of Imaging and Vice President of Support Services. All candidates will be notified of their status no later than April 15. All accepted candidates will be required to undergo criminal background checks through the Human Resources Department and the PA Department of Public Welfare. In addition, all accepted candidates must complete a full day of job shadowing at the Clearfield Hospital imaging department prior to enrollment. This shadowing experience will be scheduled by program officials.
