About Home Health
Clearfield Hospital Home Health Service has been providing the best and most efficient preventive, curative and rehabilitative home care possible for over 35 years.
Our services are available 24 hours a day through 5 offices. Our services area includes Clearfield and parts of the bordering counties of Centre, Indiana, Cambria and Jefferson.
Our agency manages an average of approximately 425 patients daily, which means we make about 51,000 visits a year, and we travel more than 600,000 miles each year to provide care to our patients.
The services that we provide include:
- In-home IV therapy
- Wound Care
- In-home rehabilitation services by our Physical Therapists and Occupational Therapists
- Speech Therapy
- Chronic and acute illness management
- Pediatrics
- Maternal/Child
- Hospice
- Social Workers
- In-Home personal care assistance through the use of certified aides

Home Health works with HomMed to provide state-of-the-art medical monitoring in the home. The Hom-Med monitoring system collects and transmits vital signs, oxygen saturation and patient weight so that a nurse can track a patient’s progress on a daily basis.
