Clearfield Hospital Home Health Service Ame Wagner, RN, Home Care Director A referral for Home Health services can be made by friends, family, community resources or physicians. The patient will undergo an evaluation to determine his or her eligibility for home care, based on need and physicians’ orders. We accept most insurances, which now have Home Health benefits and coverage. We are Medicare and Medicaid service providers and are licensed by the Pennsylvania Department of Health. |
