Clearfield Hospital Hospice Services Hospice Office What is Hospice? Persons who can no longer hope for a cure for their illness have different needs from those expecting recovery. These needs are natural, unique and specific, and deserve the most competent and compassionate care that is available to them. The Hospice team works together to help patients and families in end of life care with physical, spiritual and emotional needs. The team consists of: the patient & family, physicians, nursing staff, aides, social workers, clergy, therapists and volunteers. The Hospice Program is about choice. The patient is in control, deciding the priorities for his or her own care. The Hospice staff works hand-in-hand with the patient's own physician to provide the best care possible for the individual patient and family and to provide as much or as little care as needed or wanted. About Hospice Of primary importance in the Hospice philosophy are both physical and psychosocial needs of the patient. Hospice care focuses on the person rather than the disease, and emphasizes maximizing each day of living. As important as meeting each patients' physical needs, is the care given to emotional and spiritual concerns as well. The difficult subject of death is approached openly and honestly, helping patients and their families to address their fears and concerns related to facing loss. Hospice offers and appreciates that the loss of a loved one is painful and offers support through our bereavement program. Hospice families are supported 24 hours a day, 7 days a week, 365 days a year. Someone is always available from our Hospice staff to answer your questions and address your needs. Insurance Coverage The Clearfield Hospice Program is fully certified by Medicare and Medicaid. Most health insurance plans also include hospice benefits allowing all hospice-appropriate patients to take advantage of this service.
