Welcome to Clearfield Hospital's Pre-Registration Service.

When using the Pre-Registration service, please allow sufficient time for us to process your request. Pre-Registrations submitted a few hours before arrival may not be processed before you arrive at the Hospital.

Please keep in mind that not all hospital visits are scheduled. Some lab work may be done up to two weeks in advance, at the patient's convenience. No appointment is necessary, but registration is necessary. Therefore, all patients are required to register or pre-register, for each date of service.

When arriving at the Hospital, please bring:

1. Photo ID with current address
2. Insurance card
3. Orders and paper work from your Doctor

** Keep in mind, this is a Pre-Registration. You will need to do a final, brief registration upon arrival at the Hospital. **
